Benefits

Eligibility

To receive insurance benefits you must be a regular scheduled employee working at least 30 hours per week. Dependents eligible for medical, dental and life coverage include:

  • Your lawful spouse
  • Children to age 26 who are not eligible to enroll in an employer-sponsored health plan other than a group health plan of a parent.

Benefits Effective Date

Online benefit enrollment must be completed within 30 days of your date of hire. Insurance benefits are effective the first of the month following 30 days of continuous active employment from your date of hire. (Example: Date of hire 4/14, insurance effective 6/1)

Medical Insurance

Medical coverage is available for you and your family. You may enroll in the Preferred Provider Organization (PPO) or High Deductible Health Plan (HDHP). These plans give you choices in your medical care.

Dental

Dental coverage is available and includes preventive, restorative and orthodontia services.

401k Plan

Participation in a 401k plan provides many benefits to you and your family’s tax situation and retirement such as:

  • Immediate, 100% vesting of all company contributions
  • Company Match

Life Insurance and Accidental Death and Dismemberment (AD&D)

You are provided Basic Life and AD&D at no charge to you. The premiums are paid by RehabVisions. Basic Life and AD&D benefits equal your annual salary up to $50,000, with a minimum of $10,000.

Voluntary Long-Term Disability (LTD)

Long-term disability insurance provides protection against the loss of your income in the event of a disability that prevents you from working and earning an income.

Short-Term Disability (STD)

Short-term disability provides you with a salary continuation in the event you are unable to work for an extended period of time due to a personal medical necessity. A maximum of 65 days can be accrued. Benefits are paid at 66% of the hourly pay rate.

Voluntary Term Life (Supplemental Term Life)

This program allows you to purchase additional Supplemental Term Life insurance for you and your dependents and to pay for it through payroll deductions. The premiums for this coverage are based on age, smoker status, and amount of coverage desired. The advantage of purchasing Supplemental Term Life through RehabVisions is you receive group rates for coverage, and you may continue at the same group rate if you terminate employment with us.

Flexible Spending Account

This benefit is funded by pre-tax dollars through payroll deductions. It offers you tax-free advantages such as an increased spendable income. You choose how much you want to contribute each pay period and submit your expenses for reimbursement throughout the year. Eligible expenses are out-of-pocket expenses that are not covered under another health plan. The maximum amount you can contribute to a Medical Flexible Spending Account is $2500 per year. A debit card will be provided to you.

Dependent Care Flexible Spending Account

This benefit is funded by pre-tax dollars through payroll deductions. It offers you tax-free advantages such as an increased spendable income. A qualifying dependent is anyone under age 13 who you claim as an exemption on your federal income tax return, or a mentally or physically disabled spouse, parent or other dependent (regardless of age) who lives with you. In general, any expenses that are eligible for dependent care credit on your federal income tax return are eligible for reimbursement. The maximum amount you can contribute to a Dependent Care Flexible Spending Account is $5000 per year.

Health Savings Account

A Health Savings Account (HSA) is an individually owned, tax-free, interest-bearing savings account that is used to pay for qualified medical expenses either now or in the future. You must be covered under a qualified high deductible health plan (HDHP). You must be below Medicare eligibility age; and you cannot be covered under any other health plan. A debit card will be provided to you.

Voluntary Supplemental Benefits

These benefits are funded by pre-tax dollars. Local agents are available throughout the country for explanation and enrollment.

Vacation Days

Vacation

RehabVisions provides paid time off to all full-time and part-time employees who are regularly scheduled to work at least 20 hours per week. You will see your PTO balance after you have been employed 90 days.

Paid Holiday

RehabVisions recognizes six holidays per year. Designated holidays are New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving and Christmas Day.

Insurances

As a RehabVisions employee, you are covered under our workers compensation and professional liability insurance coverage.

Direct Deposit

Semi-monthly pay is directly deposited into your bank account.

Continuing Education Assistance

RehabVisions provides assistance through program development opportunities.

Other

Service Awards

RehabVisions recognizes the contributions made by employees, and in particular, the stability brought to the organization by the loyalty and tenure of staff. This service to the company is recognized through a formal service award program. Years recognized are 2,5,10,15,20 and 25.

Bonuses

Some positions qualify for a sign-on bonus and/or relocation package. Likewise, we have employee-referral bonuses and bonuses for recommending new clients to the organization.

Employee Referral Bonus

Bonuses are paid when candidates are referred and hired for open positions.

Contract Bonus

Bonuses may be paid when employees present leads to the organization that result in a signed contract with a new facility.
